Carol Heape earns award

Heape, MSW, CMC Fellow, was awarded the prestigious “HealthCare Hero” award by the Sacramento Business Journal in May for her work in keeping the elderly and disabled of all ages safe and comfortable in their own homes while assisting hospitals in reducing re-admission rates.

Last year, Heape helped compile information about older patients who were discharged from hospitals and readmitted in less than 30 days.

“In Northern California the hospital readmission rate is about 20 percent,” Heape said.

For those who have geriatric care managers, the rate is only about 3 percent, she said.

This is exciting news for geriatric care managers to understand that they actually keep people out of the hospital emergency room, healthy and living longer.

A certified care manager, Heape holds a master’s degree in social work and has consulted on many projects in the community.

Heape said that even people with dementia and Alzheimer’s can remain safe and at home with proper care management overseeing safety of the client, and re-assessing his or her condition.
