D’back fall sports requirements

Those interested in playing sports this coming fall at Union Mine High School need to turn in a completed Athletic Packet before attending tryouts on the first day of school, August 7.

All tryouts for football, cross country, boys soccer, girls golf, and girls volleyball begin on Monday, August 11, immediately after school. If you do not have a current 2014-15 Athletic Packet, the packets are available in the front office at Union Mine for students or parents to pick up, Monday through Friday, 8 a.m. to 3 p.m. on the front counter.

Note that the front office reopens Monday, July 14.

Packets include all the necessary forms: Parent Consent Form, Emergency Information Form, Insurance Form, as well as the Physical Form required by a physician and NOT a Chiropractor.

Athletic packets must be completed, and returned to the Athletic Secretary on July 31, August 1, August 4 or August 5.

The Athletic Office and Secretary will be open and available during those days and hours of 8 a.m.-noon, and then 1-3:30 p.m.

Should students wait until the first day of school, Thursday, August 7 to turn in their athletic paperwork, they run the risk of not being “cleared” on time to attend tryouts on Monday August 11 after school.
