D’back packets due dates

Athletes interested in playing fall sports at Union Mine High School need to turn in a completed Athletic Packet before attending tryouts for fall sports on the first day of school, August 8.

Tryouts for football, girls volleyball, girls golf, boys soccer and cross country begin Monday Aug. 12 immediately after school.

The 2013-14 Athletic Packet includes all necessary forms: Parent Consent Form, Emergency Information Form, Insurance Form, as well as the Physical Form required by a physician.

Athletic Packets must be completed and returned to the athletic Secretary on Aug. 1, 2, 5 and 6. The Athletic Office and Secretary will be open and available only during those days and hours of 8 a.m. to noon and 1-3:30 p.m.

Students that wait until the first day of school, Aug. 8, to turn in their athletic paperwork run the risk of not being cleared on time to attend tryouts Monday Aug. 12 after school.

If you have any questions, call Union Mine’s Athletic Secretary, Sandy Hamilton, at 530-621-4003, ext. 4125 after 7:30 a.m. Aug. 1-6.
