Students planning to participate in the athletic programs at El Dorado, Ponderosa and Union Mine High Schools for the 2014-15 school year are required to have a current and complete Sports Participation Health Record and updated immunizations, including tetanus and Hepatitis B.

This requirement includes a medical physician’s approval for full participation. Physicals must be done each school year.
In light of this, the three school’s Athletic Departments, in conjunction with local area physicians and nurses from the Marshall Primary Care, will hold the annual “Sports Physical Night,” Tuesday, May 13th, at the Marshall Care Center, 1095 Marshall Way in Placerville.
The cost for the physical is $10, and all proceeds benefit the athletic departments at each of the schools.
Students with the last names beginning with Q-Z report at 5:30 pm, A-F report at 6 pm, and G-P report at 6:30 pm. No appointments after 7 p.m.
